Patient has a history of asthma, allergies, smoking, heavy drinking, and asbestos exposure. Patient has been experiencing increasing difficulty in breathing over the past year and a half, with increased "rescue inhaler" usage. Patient recently started smoking again, within the past 30 days. Patient is a 35 year old male, mildly overweight, with little to no exercise and a poor diet. Patient has undergone extreme stress, as well a suicidal episode approx. 8 weeks ago.
